Heidi's breast implants are much smaller, and almost look normal on her, if only because she was so flat-chested to begin with. Likewise the shaved nose - the surgeon had a lot of material to work with. I'm not sure if Jenna's nose is the same one she was born with.
Now Heidi's gone and plumped up her lips. To be honest, her lips were almost non-existent, but with the long straight hair (extensions) she's looking more and more like Jenna with each procedure. With all the other girls from The Hills palling around without her and Spencer fending off gay porn stardom, perhaps Heidi felt a need to draw attention to herself. Or maybe she's preparing herself for the inevitable fallback career Spencer is unwilling to face.
